Grooming Form and Agreement

This is the Grooming Form and Grooming Agreement

I will need you to fill this paperwork out prior to me grooming your Fur Baby. I need it for both dogs and cats. If I do not have this paperwork filled out prior to your grooming NO SERVICE will be given!!!

I can send you this via email if you wish or you can come 15 minutes early and fill out the paperwork before I groom your Fur Baby. Please understand I need this paperwork to keep on file so I know your Fur Baby and all the important information on them.

This has been revised as of 1/2018. New one will be sent to you or giving to you upon dropping your fur baby off to be groomed. SHOT RECORDS NEED TO BE GIVEN TO KEEP ON FILE. If you do not have shots I can give shots but not rabies at this time. PLEASE LET ME KNOW AHEAD OF TIME IF THIS NEEDS TO BE DONE SO I HAVE THE VACCINE ON HAND. THERE IS A CHARGE FOR THIS FOR COST OF VACCINE AND ME GIVING IT. I HAVE BEEN TOLD IT IS CHEAPER THAN GOING TO VET BY CUSTOMERS AND THEN THEY GO TO LOCAL RABIES CLINIC.

This application can update at anytime and may be changed a little. The most recent will be given to you upon check in to fill out and sign off on.

Shots can be giving. If needed. I can give all but Rabies at this time. Price is for the vaccine and me giving vaccine. Many Customers have told me it is much cheaper to just get shots at grooming than making a vet appointment along with price of office visit. Call to ask price of what you need.
